Even though you may use professional coders, it is up to you, the medical provider of services to fully endorse, that you are using the appropriate treatment and diagnostic codes. Especially on codes you use on a daily basis. Please make sure that your medical record confirms that you are fulfilling all of the elements necessary to bill for that code, and not just some or most of them. If you have other health care providers in your practice, you should be checking about 10 of each providers charts every month to confirm this. A fresh set of eyes can make a difference during this process. Also remember, if you did not record it in your records, it’s not for purposes of insurance audits and various investigations.
